Appointment Scheduling and Punctuality

Appointments must be scheduled and confirmed through our office. We emphasize the importance of punctuality; any delays may lead to a missed appointment and the need for rescheduling. Please ensure you arrive on time to avoid any inconvenience.

What to Bring to Your Appointment

To ensure a successful appointment, please bring the following items:

• Original Government-Issued Photo ID: Such as a U.S. Driver’s License or Passport. Copies are not accepted.
• I-693 Form: Please complete the first two pages in black ink only.
• All Vaccine Records: Bring original documents only.
• English Translations: All documents must be in English.

Please allow 3-5 business days for the completion of your forms. We will contact you as soon as they are available for pick-up.

Important Note

In accordance with USCIS policy, all laboratory tests should be ordered by our authorized Civil Surgeon. Your primary care physician should not order these tests.
